This STD Awareness Month and beyond LELO and HEX<sup>\u00a0<\/sup>are supporting Center for Disease Control and Prevention\u2019s (CDC) critical &#8211; not to mention timely &#8211; message: Talk, Test and Treat.<\/p>\n<p>Because the first step towards better sexual health for all lies in understanding the risks, repercussions and reality of STIs and STDs, HEX presents the lowdown on the then, now and future of sexual health, as well as simple steps for protection and prevention &#8211; without compromising on pleasure.<\/p>\n<h2>What\u2019s in a Name?<\/h2>\n<p>STD? STI? What are they, which is worse, and what\u2019s the difference? Broadly speaking, and in keeping with World Health Organization&#8217;s (WHO) 1999 guidelines<a href=\"#_ftn2\" name=\"_ftnref2\"><sup><sup>[2]<\/sup><\/sup><\/a>, the preferred term for any number of spreadable nasties, no matter their prognosis, is sexually transmitted <em>infection<\/em> (STI). The confusion in terminology lies in the fact that an infection needn\u2019t cause adverse effects. Take human papillomavirus (HPV), for example, which CDC estimates currently infects some 79 million Americans<a href=\"#_ftn3\" name=\"_ftnref3\"><sup><sup>[3]<\/sup><\/sup><\/a>. Although linked to genital warts and, for certain virus types, cancer, for most, this sexually transmitted infection remains dormant before eventually disappearing of its own accord.<\/p>\n<p>STD stands for sexually transmitted <em>disease. <\/em>Although always preceded by an STI, the progression is far from inevitable &#8211; particularly if you get tested and treated! What\u2019s more, although an STD will affect sexual health and function, just like an STI, symptoms needn\u2019t be visible. Semantics aside, the most effective way to stop the spread of STIs and protect you and your partners is to practice safe sex: wear a condom.<\/p>\n<h2>Background: STIs in History<\/h2>\n<p>Sexually transmitted infections and diseases are nothing new &#8211; there\u2019s even speculation that the \u201c<em>sexually immoral person [sinning] against his own body,<\/em>\u201d in the Bible\u2019s 1 Corinthians could be an oblique reference to STIs. Centuries later, William Shakespeare had \u2018Troilus and Cressida\u2019\u2019s Thersites wish \u201c<em>Neapolitan-bone-ache<\/em>\u201d &#8211; bawdy Elizabethan slang for syphilis &#8211; on the entire Greek army. The particularly vicious jinx supports a widely-held theory that the disease\u2019s rapid spread from the 15th century was driven by European colonization and trade routes.<\/p>\n<p>Like today, it wasn\u2019t just syphilis that the sexually active had to contend with: in 16th century Paris, prostitutes around the Le Clapiers district were blamed for spreading gonorrhea, earning the STI its colloquial nickname still used today &#8211; \u2018the clap.\u2019<a href=\"#_ftn4\" name=\"_ftnref4\"><sup><sup>[4]<\/sup><\/sup><\/a><\/p>\n<p>Throughout the 20th century, both the US Army and Navy waged war against STIs, either through promoting condom usage (\u201c<em>Put it on before you put it in<\/em>,\u201d urged 1942 training film, <em>USS VD: Ship of Shame<\/em>), or warning soldiers against \u201c<em>procurable women<\/em>\u201d and \u201c<em>good time girls.<\/em>\u201d<a href=\"#_ftn5\" name=\"_ftnref5\"><sup><sup>[5]<\/sup><\/sup><\/a><\/p>\n<p>Fast forward to the 1960s, and human immunodeficiency virus (HIV) was already being reported. But it was the first clinical observation of late stage symptoms, acquired immunodeficiency syndrome (AIDS) in 1981<a href=\"#_ftn6\" name=\"_ftnref6\"><sup><sup>[6]<\/sup><\/sup><\/a> that propelled sexual health on to the public radar, mixing messages of stigma and safety, treatment and taboo.<\/p>\n<h2>STIs Today &#8211; And Tomorrow?<\/h2>\n<p>STIs may be old news, but that\u2019s not to say that they\u2019ve stayed the same. Like all living things, the bacteria, viruses and parasites that cause STIs evolve and adapt for survival. Such is the case with what\u2019s ominously being referred to as \u2018super-gonorrhea,\u2019 currently causing concern in the UK in particular<a href=\"#_ftn7\" name=\"_ftnref7\"><sup><sup>[7]<\/sup><\/sup><\/a>. Long successfully treated with a combination of ceftriaxone and azithromycin, a current strain is proving resistant to the latter antibiotic. The second most common STI globally, left untreated gonorrhea can cause infertility, pelvic inflammatory disease and even blindness in infants born to infected mothers.<\/p>\n<p>Meanwhile, in Central and South America, as well as areas of the Caribbean and beyond, a more complex threat still is gathering pace. Primarily spread via mosquitoes, Zika virus is not classified as an STI. Nonetheless, we now know that the virus can be transmitted sexually by men &#8211; with potentially devastating consequences. Just this month, a CDC review confirmed what had long been suspected: Zika virus is a cause of microcephaly and other severe fetal brain defects<a href=\"#_ftn8\" name=\"_ftnref8\"><sup><sup>[8]<\/sup><\/sup><\/a>. With that in mind, WHO\u2019s latest guidelines regarding the sexual transmission of the virus is clear: \u201c<em>All people who have been infected with Zika virus and their sexual partners\u2014particularly pregnant women\u2014should\u2026 have access to condoms and use them correctly and consistently<\/em>.<a href=\"#_ftn9\" name=\"_ftnref9\"><sup><sup>[9]<\/sup><\/sup><\/a>\u201d<\/p>\n<h2>2016: The Summer of Safe Sex<\/h2>\n<p>When it comes to Zika, it\u2019s not just individuals living in affected regions who are at risk. As the Rio 2016 Olympic Games in Brazil draw ever closer, spreading the safe sex message is becoming more urgent &#8211; not least among the 10,500 athletes set to compete. For when all those endorphins, testosterone and sports-honed bodies become irresistible, at the London 2012 games a whopping 150,000 condoms were made available to athletes; with winter\u2019s Sochi 2014 proving similarly sizzling.<\/p>\n<p>Of course, it\u2019s easy to make light of the titillating track record of what happens when sports and sex hook up &#8211; but the message is nonetheless a serious one, and ensuring availability of a condom not just to suffice, but to inspire, is essential.<\/p>\n<h2>Spread the Word &#8211; Not STIs!<u> <\/u><\/h2>\n<p>In the same way STIs are nothing new, our approach towards today\u2019s pandemic is similarly outdated. Discussing sexual histories matters. What are their thoughts on condom usage? When did they last get screened? Finally, if the prospect of honest exchange leaves you blushing, should you really be getting intimate with this person?<\/li>\n<li><strong>Test<\/strong>: Because the majority of STIs present no tell-tale symptoms, the only way to know for sure is to get tested. Talk to your local health provider, and relax: depending on what you\u2019re being tested for, you\u2019ll either have a quick blood test, swab of genital areas, or be asked to provide a urine sample. Easy!<\/li>\n<li><strong>Treat<\/strong>: Bacterial STIs &#8211; chlamydia and syphilis, for example &#8211; can be quickly treated with a course of antibiotics prescribed by your doctor. The symptoms of viral STDs such as HIV can be managed with medications.
